Four MS Word forms that record how an organisation manages its use of artificial intelligence - the life cycle of each AI system, the data used to build it, the impact of using it on the people it affects, and the committee that keeps the whole arrangement under review.

Nine numbered stages recorded for each AI system, from requirements and specification through resources, verification and validation, deployment, operation and monitoring, technical documentation and event logs, to closure. One sheet per system.
The data behind an AI system - what is required, how it is acquired and selected, the quality it must meet, how provenance is recorded across development life cycles, and the preparation criteria and methods applied.
The AI system impact assessment required by clause 6.1.4. A six-question screening section decides whether a full assessment is needed, then four stages cover use of the system, who is affected, how they are affected including foreseeable misuse, and the controls, residual impact and human oversight. Closes with a decision, conditions of use, disclosure and sign-off.
The committee record for reviewing AI arrangements and systems in use, emerging capabilities and threats, with issues, suggestions and recommended actions. Its guidance note is included in the same file.
